Central limitationThe records establish procurement, shipment, installation, repair, and maintenance, but do not identify patients or prove that any procedure occurred

The dental treatment room on Jeffrey Epstein’s Little Saint James island was not merely a room that happened to contain a dental chair. Released records document the purchase, customization, international shipment, placement, plumbing, compressor installation, suction equipment, electrical work, repair, and continuing maintenance of the room.

The procurement trail begins in June 2013, when Karyna Shuliak purchased a computer controlled dental chair through eBay and requested seamless yellow orange upholstery. Shipping records directed the unit through freight forwarders to LSJE, LLC, on Little Saint James. Epstein subsequently instructed employees to place the chair in “Sarah’s office on LSJ.”

Later records call the space “Karyna dental office.” Property work lists document a compressor, suction system, dental vacuum, chair motor, plumbing problems, electrical work, a requested opening through the floor, and a final switch installed by a worker identified as Mr. Plasket. In 2015, Shuliak purchased a dental cabinet with a sink and mobile cart for $2,700. Epstein then instructed island workers to assemble it in Karyna’s dental office.

Federal investigative photographs taken on August 12, 2019, show the resulting room. The images establish that a private dental treatment facility existed and that significant equipment had been purchased and installed. They do not establish who received treatment, who performed procedures, whether the room operated as a licensed dental practice, or whether it had any role in Epstein’s criminal conduct.

What Is This Record?

This article concerns a connected body of purchasing records, shipping documents, property work lists, maintenance communications, and investigative photographs related to the dental treatment room on Little Saint James.

The principal photographic sequence appears in EFTA00001853, EFTA00001854, EFTA00001855, EFTA00001856, EFTA00001857, EFTA00001858, EFTA00001859, and EFTA00001860.

A related congressional series consists of HOUSE_OVERSIGHT_034651, HOUSE_OVERSIGHT_034652, HOUSE_OVERSIGHT_034653, HOUSE_OVERSIGHT_034654, HOUSE_OVERSIGHT_034655, HOUSE_OVERSIGHT_034656, HOUSE_OVERSIGHT_034657, HOUSE_OVERSIGHT_034658, and HOUSE_OVERSIGHT_034659.

The photographs show a yellow dental chair, overhead treatment lighting, cabinetry, a sink, work surfaces, storage, woven baskets, an adjoining area, and distinctive wall mounted faces. No patient, clinician, treatment procedure, dental chart, consent form, prescription, sterilization log, or treatment record is visible.

The purchasing and installation records provide context that the photographs alone cannot supply. They show that the room was intentionally equipped over several years and that Epstein personally participated in decisions about its location, equipment price, installation, repairs, and electrical safety.

The Dental Chair Purchase

A PayPal receipt in EFTA00576673 documents the June 22, 2013 purchase of a dental chair by Karyna Shuliak.

The receipt identifies the product as a “New Dental Unit Chair Computer Controlled A1-1 Model FDA CE hard leather.” It lists eBay item number 251235812585 and records a payment of $1,790.

The shipping information names Roy Hodges and identifies Tropical Shipping as the freight forwarder for LSJE, LLC, Little St. James. The order includes the customization request, “Seamless yellow-orange cover. Include all listed accessories please.”

The requested yellow orange upholstery is visually consistent with the yellow chair later photographed in the Little Saint James dental room. The similarity is significant, although a definitive equipment match would require a serial number, manufacturer plate, or model number visible in the photographs.

In EFTA00538865, Shuliak supplied a photograph of the selected upholstery color and again requested seamless upholstery. The seller asked for information concerning the destination seaport, customs clearance, telephone contact, and freight forwarding to LSJE, LLC.

Additional correspondence in EFTA00529160 identifies the same eBay item and a seller located in China. The seller discussed the payment, customs requirements, and an ocean shipping period of approximately one to two months.

Together, these records establish the chair model, purchase price, purchaser, upholstery request, international seller, freight route, and intended Little Saint James destination.

Shipping the Chair to Little Saint James

The dental chair’s shipping trail appears across several released records.

On June 24, 2013, Shuliak informed Daphne Wallace, Stephanie Hodges, and Roy Hodges that the dental unit “needs to be shipped to LSJ.” EFTA02302245 identifies Tropical Shipping and the LSJE, LLC freight forwarding arrangement.

A July 23, 2013 email in EFTA00529478 carries the subject “B/L of the dental chair unit, from Lisa” and references an attached bill of lading.

The following day, EFTA00529972 documented the scheduled July 26 departure and discussed the packing list, commercial invoice, customs broker, and importer security filing.

Customs correspondence continued in EFTA00539050, which concerns the original bill of lading and forwarding documentation to the customs broker.

EFTA00529194 lists the destination as Roy Hodges, LSJE, Little St. James Island, 6100 Red Hook Quarters B3, St. Thomas, United States Virgin Islands.

By September 11, 2013, EFTA00535139 reported that the chair had arrived in New York. Customs paperwork and fees had been completed, and transportation to Miami was scheduled.

The records therefore document a planned route from the overseas seller through United States customs, New York, Miami, Tropical Shipping, St. Thomas, and ultimately Little Saint James.

Conversion of Sarah’s Office

The most important location record appears in EFTA02297530.

On September 29, 2013, Shuliak wrote that “Mr. Epstein advised, that the dental chair should be placed at Sarah’s office on LSJ please.” Ann Rodriquez responded, “Will do.”

The message indicates that the chair was not initially destined for a room already identified in the correspondence as a dental office. It was to be placed in an existing space known as Sarah’s office.

By September 2015, Epstein referred to the same project as “karyna dental office” in EFTA00845854. This change in terminology supports the conclusion that an existing office was converted into a dental treatment room associated with Shuliak.

The records do not identify which Sarah used the office, when that person stopped using it, or whether the conversion required structural permits. The evidence does establish Epstein’s role in selecting the location.

Compressor, Plumbing, Suction, and Electrical Work

The chair required more than delivery and assembly. Property records document supporting mechanical and electrical systems.

EFTA00632208 records “Compressor for Dental Office” as completed on October 30, 2013.

Two days later, EFTA01753512 marked “Dental chair ready for testing” as completed on November 1, 2013.

In EFTA02577146, property staff explained that the dental compressor had a built in automatic switch that controlled operation when the equipment was in use.

Problems appeared during the following year. On April 21, 2014, Epstein reported a water leak in the dental office and directed Ann Rodriquez to contact a dentist in St. Thomas to find someone who understood how to set up the chair and shut off its water supply. The instruction appears in EFTA01925439.

Correspondence in EFTA00663052 states that a dental technician was expected to visit the island to determine whether the chair had been assembled correctly.

On July 24, 2014, Rodriquez asked Epstein for authorization to purchase a suction system and dual compressor for $4,000, including installation. The proposal appears in EFTA01793473.

Epstein responded “bid 2500” in EFTA01917134. Rodriquez answered “Done” in EFTA01917980. Her response establishes that she acted on Epstein’s direction. It does not establish that the vendor accepted the $2,500 offer or that this was the final amount paid.

By July 29, 2014, EFTA02586652 reported that the dental compressor and suction system would be installed on Little Saint James that Friday.

A property project list preserved in EFTA01309317 calls for purchasing a dental compressor and dental vacuum system. It also requests permission to make a hole through the floor. The available text is partially truncated, but the entry places floor modification directly within the dental installation project.

The same project record identifies electrical work in the dental office and states that Mr. Plasket was expected to complete the installation so the chair would be ready for use. The work was listed as 90 percent complete with a November 18, 2014 completion date.

On September 15, 2014, Epstein issued the instruction “install dental chair motor” in EFTA00996912. The same exchange continues in EFTA00996913.

On December 11, 2014, Rodriquez reported that Mr. Plasket was onsite “putting on the final switch on the Dental chair.” That communication appears in EFTA02597607.

These records document a meaningful construction and installation process involving plumbing, a compressor, suction equipment, a vacuum system, a motor, a floor opening, electrical work, and a final control switch. They do not reveal whether a building permit or professional dental facility permit was obtained.

Repairs and Technician Visits

The installation required continuing repairs.

On May 28, 2015, Shuliak reported several defects in EFTA00536542. She stated that the chair’s bottom panel had separated, the water was not working, and the compressor was making excessive noise. She asked Rodriquez to have Danny repair the equipment.

The longer correspondence in EFTA00536151 documents efforts during August 2015 to make the dental room ready for use. Rodriquez reported that another technician had asked about a business license and a license to practice in the United States Virgin Islands.

The correspondence does not show an answer to those licensing questions. It therefore does not prove that the facility was unlicensed. It establishes only that a prospective technician raised the issue.

The same thread reports that Mr. Plasket eventually repaired and reassembled the chair. A dental cabinet was also scheduled for pickup.

EFTA00707518 records another planned visit by Mr. Plasket to work on the dental office in August 2015.

The released records do not fully identify Danny, Mr. Plasket, or the outside dental technician. Their qualifications, employers, invoices, and complete work histories remain unresolved.

The Dental Cabinet Purchase

Shuliak purchased additional furniture for the room in 2015.

EFTA00530394 records a June 20, 2015 eBay order for an “LS1 Dental Clinic Cabinet w/Laminate Top, Sink 3 Module and Mobile Cart.”

The purchase was initially directed to 358 El Brillo Way in Palm Beach. A corresponding PayPal invoice in EFTA00535986 identifies Certified Dental Supply as the seller.

The invoice shows:

ItemAmount
Dental cabinet$2,200
Shipping$500
Total$2,700

The invoice identifies the recipient as Dr. Karyna Shuliak and records payment using an American Express card ending in 1088.

EFTA00574551 states that the cabinet was expected to sail from Florida on August 7, 2015, and arrive in St. Thomas on August 11. The correspondence requests the commercial invoice and country of origin for shipping purposes.

On September 18, 2015, Epstein instructed Rodriquez to have workers “put together the cabinet in karyna dental office.” The order appears in EFTA00845854 and its duplicate production record, EFTA00698758.

This sequence connects the cabinet purchase to the Little Saint James dental room even though the initial invoice used Epstein’s Palm Beach address.

Confirmed Costs

The released receipts establish at least $4,490 in completed purchases.

EquipmentDocumented amountStatus
Computer controlled dental chair$1,790Paid
Dental cabinet, sink modules, and mobile cart$2,200Paid
Cabinet shipping$500Paid
Suction system and dual compressor$4,000 proposalProposed

The known paid total should not be treated as the total cost of the room. It excludes the treatment light, compressor, suction system, vacuum system, chair motor, electrical work, plumbing, freight handling, customs charges, installation labor, repairs, wall decor, and any instruments or supplies not identified in the available receipts.

Continuing Maintenance and Safety Instructions

The room continued to appear in property operations after the major installation period.

On November 22, 2016, Epstein instructed staff to ensure that the dental chair pump was unplugged. Brice Gordon responded, “Got it.” The exchange appears in EFTA01060767 and EFTA02669359.

A September 2016 property communication in EFTA00818480 describes a large room beneath the dental office containing air conditioning equipment and a black cabinet with a Cisco switch. This provides a clue about the dental room’s physical position within the building, although it does not establish the purpose of the space below.

A mandatory Little Saint James project list dated March 23, 2017, instructed workers to check all wiring and make sure nothing was running on the dental chair. That instruction appears in EFTA00677705.

These records indicate that the chair remained connected to active mechanical or electrical systems and required continuing attention years after its original purchase.

What the Photographs Contain

The investigative photographs show the completed room and its visible contents.

Visible featureSupported descriptionEvidentiary limitation
Yellow treatment chairConsistent with the customized yellow orange chair ordered in 2013No visible serial number definitively matches it to the receipt
Overhead lightArticulated clinical treatment lightPurchase record has not been located
CabinetryDental storage and work surfacesNot every cabinet can be matched to a specific invoice
Sink areaConsistent with the cabinet and sink modules purchased in 2015Plumbing specifications are not visible

The yellow chair visible in the photographs is strongly consistent with Shuliak’s request for a seamless yellow orange covering. The appearance, room destination, and documented installation create a substantial circumstantial match. A serial number or manufacturer plate would still be required for definitive identification.

The wall mounted faces remain unexplained. No purchase receipt, artist record, catalog entry, or reliable identification has been located. Claims that the faces depict specific public figures or were created from real people remain unsupported.

Shuliak’s role is no longer limited to speculation based on her dental education. The records directly connect her to selecting, purchasing, shipping, inspecting, and seeking repairs for the equipment. They also document Epstein calling the space “karyna dental office.”

These records do not independently establish that Shuliak treated patients in the room. They establish her direct involvement in creating and maintaining it.

Independent reporting by the Butterfly Bureau has examined Shuliak’s dental history, the Zorro Ranch equipment trail, and Epstein’s support for dental infrastructure. American Freakshow’s reporting provides additional context concerning Shuliak’s relationship with Epstein and her later role in his estate. Neither investigation should be treated as a substitute for the direct EFTA records, but both provide relevant context for the broader dental trail.

Reliability and Evidentiary Limits

The receipts and email records have high evidentiary value for purchases, instructions, shipping, and reported work. They identify specific products, prices, participants, destinations, and installation tasks.

Property status lists are useful for showing what staff reported as completed. A “done” notation does not always establish that work was professionally performed, inspected, paid for, or completed according to building and dental standards.

The $4,000 compressor and suction proposal is documented. Epstein’s $2,500 counteroffer is also documented. The final accepted price has not been established.

The placement email strongly supports conversion of Sarah’s office into the dental room. A formal construction plan, permit, contractor invoice, or architectural drawing specifically naming the room has not been located.

The photographs reliably establish visible room contents in August 2019. They do not establish how frequently the equipment was used or whether it remained fully operational.

No publicly identified patient chart, appointment book, consent form, treatment invoice, radiograph, sterilization log, prescription record, or medical waste record has been connected to the room.

The licensing discussion must be handled carefully. A technician asked about a business license and a professional license. The available thread does not contain the answer. It is therefore inaccurate to claim that the room was licensed or unlicensed based on that exchange alone.

Survivor Safety and Privacy Review

Little Saint James is connected to documented survivor accounts, civil litigation, and allegations of trafficking and sexual abuse. That history makes unexplained private clinical infrastructure a legitimate subject of investigation.

The evidence does not establish that any survivor was treated in the room. No survivor should be identified as a patient without a public statement from that survivor or reliable documentary evidence that can be reported without compromising privacy.

Claims about forced dental procedures, altered identities, extracted teeth, involuntary sedation, or experimentation remain unsupported by the records cited in this article. Publishing those theories as fact could distort survivor testimony and weaken evidence based investigation.

The responsible investigative focus is the paper trail: purchases, freight records, contractors, licensing, professional qualifications, patient documentation, treatment schedules, waste disposal, medications, equipment serial numbers, and government search inventories.

Why This Record Matters

The dental room matters because the combined records reveal a planned and maintained clinical space on Epstein’s secluded private island.

The room required purchasing, international shipping, customs handling, plumbing, electrical work, a compressor, suction equipment, a vacuum system, a chair motor, cabinet assembly, technician visits, repairs, and continuing safety checks. That infrastructure created multiple opportunities for documentation and outside witnesses.

The records also demonstrate Epstein’s personal involvement. He chose the room, negotiated equipment pricing, ordered motor installation, monitored repairs, directed cabinet assembly, and later instructed staff to disconnect the pump and inspect the wiring.

Shuliak’s documented role is similarly substantial. She selected and purchased equipment, specified upholstery, coordinated shipping, reported defects, and asked whether the room was ready. The later description “Karyna dental office” directly connects her name to the facility, although it does not prove that she treated patients there.

The remaining question is no longer whether the room existed or whether its equipment was intentionally installed. Those points are supported. The unresolved questions concern licensing, professional operation, patients, procedures, records, and the purpose of maintaining a private dental facility on Little Saint James.
